To open the questionnaire, go to the Intake tab and click on the form listed. This can be done at anytime to view progress even if the client has not submitted the form.
To resend a questionnaire, click on the 3 dots to display the menu, and click 'Resend'.
If you use the resend button, this will reset access to the Questionnaire if your client has previously submitted it.
To complete or submit the questionnaire on behalf of the client, simply click on the form or using the 3 dots menu icon, choose 'Edit Responses'. This will open the questionnaire where you can go to the last page and click 'Submit'. This will prevent the client from accessing and editing the questionnaire from this point.
Note: This is an important step. The matter can now be managed and edited within the Property and Parenting tabs in Smokeball.
Form Status
Internal Use - The form has been selected and is being used internally (not yet submitted)
Shared - The form has been shared with the client.
Completed - The form has been submitted by the client or internally by the practitioner
